I acknowledge that I have voluntarily chosen to participate in the courses offered by Lujan Matus, Safe Haven Holistic Center and Parallel Perception Limited. I have full knowledge of the risks that these activities present, including travel to, participation in, and returning from the activities or events.
I understand that by being permitted to participate in Lo Ban Pai tuition, I agree to assume any and all risks of injury or death. I further understand and agree to assume responsibility for all risk of theft, loss, or damage of personal property which occurs at any time arising out of my participation in activities with Lujan Matus, Safe Haven Holistic Center and Parallel Perception Limited.
I understand and agree that as a condition of participation in activities and tuition, I further agree to release liability and indemnify Lujan Matus, Safe Haven Holistic Center and Parallel Perception Limited, the officers, directors, agents, employees, assigns, successors, or lessors from any damage, injury, or death to myself or to any person or property, in any way connected with my participation, and tuition received from Lujan Matus, Safe Haven Holistic Center and Parallel Perception Limited.
I understand that I should consult a physician prior to beginning any activity program, including Lo Ban Pai. I recognize that it is my responsibility to notify Lujan Matus, Safe Haven Holistic Center and Parallel Perception Limited of any serious illness or injury before every class and take full responsibility if I decide to participate with an existing injury.
I understand that Lo Ban Pai consists of physical activity, and that injuries may occur during the normal course of class activities.
Electronic Devices
Under no circumstances are any electronic devices permitted into the tuition venue, including cellphones, tablets or laptops. No exceptions will be made to this requirement. You are advised to leave your electronic devices and other valuable items either in the safe in your hotel room or in another secure location.
Copyright and Non-Disclosure
I agree that all movements learned during the course are for personal use only and are not to be demonstrated or taught to others under any circumstances. Therefore, I will not teach, demonstrate or share any parts of workshops or private tuition with others, either orally, in writing, or through any media. I also will not ask another student to share their notes or demonstrate the movements.
I agree not to record workshops, private tuition, and online sessions in any format, including video and audio. I also agree not to take videos of myself or other students performing the movements during or after the course. A breach of this agreement will be considered as infringement of copyright. I further agree not to comment, write, or post any pictures about workshops or private tuition on any media, including social media.
I agree that attendance at any workshop or private tuition program does not qualify me to teach, demonstrate, video the movements or share notes about Lo Ban Pai – in part or full – under any circumstances. Failure to adhere to these requirements will result in a ban from participation in future workshops and private tuition and is considered a violation of the copyright of Lo Ban Pai, of which Lujan Matus is the founder.
Email Solicitation
Under no circumstances is anybody in any workshop permitted to obtain the group’s emails in their entirety. Distributing business cards or other advertising materials to the group is also prohibited. If anyone is found collecting emails or distributing business cards or advertising materials, they will be removed from the workshop immediately and banned from any further participation in Lo Ban Pai.
Lujan Matus, Safe Haven Holistic Center, and Parallel Perception Limited reserve the right to decline to accept or retain any person whose actions, physical health, mental condition, or general deportment impede workshop facilitation or rights, welfare, or enjoyment of other workshop members. At any time during private tuition, Lujan Matus, Safe Haven Holistic Center, and Parallel Perception Limited reserve the right to discontinue tuition if the student is deemed unsuitable for the training.
Participation in courses offered by Lujan Matus, Safe Haven Holistic Center, and Parallel Perception Limited requires completion of the registration form. Students will not be permitted to attend tuition until this form has been submitted with their full legal name. If a false name or alias is used, the student will be disqualified from participating in any of the courses with Lujan Matus, Safe Haven Holistic Center, and Parallel Perception Limited, and no refund will be permitted.
Travel Insurance
We advise all participants to purchase a comprehensive travel insurance plan that will reimburse tuition fees and travel expenses in case of unforeseeable circumstances. If a student fails to do so they will be responsible for any costs incurred due to cancellation of the event or any other unforeseeable circumstances, including natural disasters and acts of war.
In no situation is Lujan Matus, Safe Haven Holistic Center, and Parallel Perception Limited liable for losses due to inability to attend tuition and subsequent loss of tuition fees, non-refundable travel expenses, including flights and accommodation, or medical costs.
Cancellation
If the participant cancels their tuition or is unable to attend under any circumstances, including being declined by Lujan Matus, Safe Haven Holistic Center, and Parallel Perception Limited, refunds are not available. The no-refund policy applies regardless of whether the registration form has been completed or not.
Tuition fees may be transferred to a future date or utilized for other activities (including online tuition) that are offered by Lujan Matus, Safe Haven Holistic Center, and Parallel Perception Limited if at least four months’ notice is provided.
This does not apply to large group workshops and gift cards. All group workshop fees are non-refundable and non-transferable due to the fact that the workshop venue and associated costs are paid in advance. Gift cards are valid for one year from the date of issuance and are non-transferable and non-refundable.
Any rescheduled program must be completed within a year of the cancellation date; otherwise, the tuition fee is forfeited, and no refund or future rescheduling will be available.
If you need to reschedule an online appointment, please give us 24 hours’ notice. That week’s appointment will be forfeited if you miss a scheduled call.
Legal Agreement
I understand and agree that I have carefully read this agreement and fully understand all of its terms and conditions. I understand that this is a release of liability which could legally prevent me from filing suit or making any other claim for damages in the event of my death or injury to me. With this knowledge, I am entering into this agreement freely and voluntarily.
I agree that this is binding on myself, my assigns, and legal representatives. I understand and agree that I have read all of this waiver and agree to all the terms and conditions above.
